Хто хо́че жи́ти ві́чно? ― Xto xóče žýty víčno? ― Who wants to live forever?

The verb бажати (to desire) is not used as a polite form of хотіти (want); хотіти can be softened by using its past form хотів, which is equivalent to would like.
