One book usage [1] The Steroid Deceit: A Body Worth Dying For? By Jeff Rutstein , Custom Fitness Publishing, LLC, Aug 31, 2005. AFAICT, the publisher is Rutstein himself - and I found no other usages. I suspect that therefore there might not be 3 independent usages.
