I think it's natural in English to drop it in direct address. Note that—until I just changed it to bring it in line with your requests here—Madonna didn't dump the article into the head. It just put a lb tag on the sense. — LlywelynII 09:36, 7 May 2023 (UTC)Reply

@LlywelynII: Because Madonna is only sometimes used with the (in the third person), unlike Blessed Virgin. Putting “sometimes” in the headword is not an option. J3133 (talk) 09:49, 7 May 2023 (UTC)Reply
... but Blessed Virgin is only sometimes used with "the" (in the third person) in exactly the same manner and "(the)" precisely means "sometimes" in the headword.
