Literal Meaning[edit]

The metaphorical use is prolific, but I actually saw the literal circumstance of a door that had "come unhinged" the other day, or at least partially unhinged. One of two of the hinge mechanisms rusted out of the wall, and the door could be opened and closed but was unstable.
