תּוֹךְ (tókh, inside) +‎ ־וֹן (-ón). Compare רִאשׁוֹן (rishón, first) from רֹאשׁ (rósh, head).

The sense high school is an ellipsis of בֵּית סֵפֶר תִּיכוֹן (high school), and the sense median is a semantic loan from English.
