1.) The etymology says "many of the adverbs", while the definition says it has to be a word with "wh". So is a question with "how", for example, a "wh-question" or not? (Of course, "how" is from Proto-Germanic *hwō, but that's beside the point; it simply doesn't begin in "wh".).

I am not a linguistics expert, but according to the English Wikipedia article "w:Question" it does appear that a question beginning with how is regarded as a wh-question: see "w:Question#By grammatical form".
