John Camden Hotten's Slang Dictionary (1873) says that to "play the Scotch fiddle" was to work the index finger of the right hand like a fiddlestick between the index and middle finger of the left. This gesture was used to insult Scots by implying that they had the itch. I can't find much in the way of citations. Equinox 00:48, 12 April 2021 (UTC)Reply
