Etymology

From a reanalysis of quid nōmen tibī̆ est (what is your name?), where the pronominal interrogative quid (what) was hypercorrected to the adnominal determiner quod (which), perhaps caused by the progressively increasing confusion between the two paradigms. The resulting literal meaning is "which name do you have?", or more idiomatically "which is your name?".
