Etymology

Effectively an action noun of dialectal Bulgarian осипя (osipja, “to secrete, to pour”), from Proto-Slavic *osъpa.

The name of the disease comes from the papules caused by the Variola virus. For the semantic shift, compare Bulgarian оток (otok, “edema, swelling”) (from Bulgarian отичам (otičam, “to flow out”)).
